When an entire episode is devoted to Kagura's father fighting to protect some alien world, even achieving a level of drama, and in the end turns out to be one long commercial for a hair loss treatment.

Mayushii getting killed repeatedly, over and over, in different manners, in multiple episodes, no matter how many times Okarin time leaps to save her.

On peaceful beautiful morning, like any other, "Godou Kusanagi" wakes up by alarm clock, realizes he's late, quickly starts to fix himself up. But than his world starts falling apart when he sees that he might have "nailed" a reall beauty, what is totally out of his normal behaviour (he slept with one of the most beautiful girls "Erica Blandelli", without him even clearly remembering anything about how, when or what happened that she ended in the same bed as he himself... :fail

My most WTF moment would definitly be the following:
